Make an appointment

To make an appointment, we can be reached by telephone from Monday to Friday from 8:30 am to 5:00 pm.

First visit

You can complete the registration form on this website or you can complete the registration form at the practice when you come for your first appointment. Please arrive 15 minutes before your appointment so that we can complete your registration at reception. 

Please bring:

• Identity card with photo, or passport, or driver's license or other ID.
• You also take your health insurance card with you.

You will be required to complete a health questionnaire so that the dentist can provide you with appropriate care. Make sure you fill this out carefully.
